About the Role

As an NDIS Service Coordinator, you'll play a vital role in supporting our Supported Independent Living participants. Working closely with the Service Delivery Team, you'll help ensure that each participant receives person-centred, high-quality support. You'll collaborate with disability support workers, participants, families, stakeholders, and support teams to coordinate services that align with individual goals and care needs—making a meaningful difference in their lives.

The Roster: Flexible Options to suit your lifestyle

Part time opportunity, approximately 24 hours per week

Key areas of focus include

  • Participant safety
  • Care planning
  • Stakeholder communication
  • Quality assurance
  • Coaching and leadership of support workers

You'll also contribute to risk management, incident resolution, and continuous improvement—helping shape a safe, responsive, and accountable service environment.

Key Accountabilities:

  • Assist in the development and implement person-centred care plans
  • Brief and support disability support workers to deliver quality care
  • Maintain accurate documentation and ensure compliance
  • Respond to complaints and incidents professionally
  • Participate in the on-call roster
  • Conduct risk assessments and support medication compliance
  • Provide direct support for participants when needed

What We're Looking For:

  • Certificate III in Individual Support (Disability) or higher (or equivalent experience)
  • Experience in direct support work within Supported Independent Living
  • Strong understanding of NDIS frameworks and safeguarding principles
  • Proficiency in client management systems and digital tools
  • Empathy, adaptability, and emotional resilience
  • Strong communication and problem-solving skills
  • Team collaboration and accountability
  • Cultural sensitivity and discretion
  • Proven ability to lead and guide teams effectively

Additional Requirements:

  • Current First Aid & CPR, Working with Children Check, and NDIS Worker Screening Check
  • Medication assistance training
  • Completion of the NDIS Worker Orientation Module
  • Valid driver's licence and access to a reliable vehicle
  • Manual handling and infection control training (desirable)
