Team Leader/NDIS Support Coordinator – Work with Purpose

Are you a skilled and experienced NDIS Support Coordinator looking to take the next step in your career, while staying connected to the people and communities you care about?

At PCCS, we believe in walking alongside people as they navigate the NDIS and work toward their goals. As a Team Leader/NDIS Support Coordinator, you'll play a hands-on role in supporting participants with complex needs to get the most out of their plans, while also sharing your experience to strengthen and guide our support coordination team.

This role is ideal for someone who enjoys direct client work, thrives on problem-solving, and is passionate about making services more accessible, responsive, and empowering people.

What you'll be doing:

  • Providing high-quality, person-centred support coordination to NDIS participants with complex needs
  • Building strong, trusting relationships with participants and their support networks
  • Supporting participants to understand and implement their NDIS plans effectively
  • Identifying barriers and working collaboratively to find creative, sustainable solutions
  • Coaching and guiding junior support coordinators, sharing your expertise in a supportive role
  • Contributing to service improvement by identifying gaps and opportunities across the coordination team

You'll love this role if you:

  • Hold a degree in Social Work and are eligible for membership with the AASW
  • Have strong experience delivering NDIS Support Coordination, particularly with participants with complex needs
  • Bring a deep understanding of the NDIS and how to effectively connect people with the right services and supports
  • Enjoy empowering others, whether it's the participants you support or the junior team member's you mentor
  • Are collaborative, values-driven, and passionate about person-centred, strengths-based practice
  • Want to work in an organisation that celebrates innovation, inclusion, and impact
  • Valid NDIS Worker Screening Check and Working With Children Check
  • Have a current driver's licence and access to a reliable vehicle
