We are seeking an experienced Registered Nurse to assess individuals in home and community settings to join our South Eastern Suburbs Assessment Services team covering several local government areas (Port Philip, Stonington. Boroondara, Whitehorse, Glen Eira and surrounding LGA's)
The successful applicant is primarily responsible for receiving referrals and undertaking holistic assessments and reviews for clients within the Home and Community Care Program for Younger People (HACC-PYP) and other funded services. The position will also undertake nursing services within the community as required.
The ideal candidate will be an experienced Community Nurse Assessor, RAS or ACAS assessor who also possess the ability to assess their client's care requirements in the context of their psychosocial and functional needs. *What you bring:*
- Experience undertaking home and phone based holistic assessments as a Registered Nurse Division 1
- APHRA Registration
- Previous experience within a HACC-PYP (or other funding) environment and familiarity with the community services network.
- A good understanding on research methods of locating local services for clients and making appropriate referrals. Demonstrated experience in working with people and/or persons with a disability, in areas of assessment, care planning and care co-ordination.
- Ability to engage clients from diverse backgrounds and establish rapport.
- Strong IT skills including Microsoft Office suite.
- Ability to work autonomously including being well organised and highly developed time management and prioritisation skills.
- Current Victorian Driver's Licence.
- NDIS Screening check + Orientation module
- Knowledge of Occupational Health and Safety principles and systems
