Community Health Nurse

We are seeking a compassionate and skilled Registered Nurse to deliver high-quality patient care in the community setting.
About the Role:- Conduct comprehensive clinical assessments, develop clinically appropriate care plans, and provide a range of nursing services including medication management, wound care, and cognitive assessments.
- Work collaboratively with healthcare professionals to achieve positive patient outcomes and educate patients and their families to optimize their ability to live safely within the community.
- Develop and implement holistic care plans that meet the unique needs of each patient.
This is an exciting opportunity for a dedicated and motivated Registered Nurse who is committed to delivering exceptional patient-centered care in a fast-paced environment.
Key Requirements:- Unrestricted registration with AHPRA as a Registered Nurse.
- Availability to work on Tuesdays and Thursdays.
- Sound clinical knowledge and skills, with experience working with patients who have chronic and complex health needs.
- Excellent verbal and written communication skills, with the ability to work effectively with patients, families, and other healthcare professionals.
