Home Care Nurse - Mornington Peninsula Position
About the Role:A skilled and compassionate District Nurse is sought to join our team in providing exceptional patient care within the Mornington Peninsula area.
Key Responsibilities:- Conduct thorough clinical assessments and develop tailored care plans for clients
- Provide a range of nursing services including medication management, wound care, diabetes management, catheter care, stoma therapy, and cognitive assessments
- Collaborate with other healthcare professionals to deliver a holistic approach to patient care
- Educate patients and their families to enhance their ability to live safely within the community
- Work collaboratively with case managers and support coordinators to achieve positive patient outcomes
- Unrestricted registration with AHPRA as a Registered Nurse
- Availability on Tuesdays and Thursdays
- Sound clinical knowledge and skills
- Experience working with patients who have complex health needs
- Experience working with patients from diverse cultural and socio-economic backgrounds
