About the Role

The role of the Early Years Community Connector is to coordinate health, wellbeing and developmental supports for CALD children aged 0 to 8 years presenting with developmental delay and/or disability. The position outcomes include addressing complex barriers to accessing relevant services by providing practical support, advocacy, information, referrals, and case coordination to ensure collaborative, holistic supports of children and their families. They are employed to respond to Logan Community Hub referrals as well as those identified across SSI programs.

Key responsibilities will include:

  • Support families to access quality play, learning experiences, healthcare and early intervention
  • Collaborate with CALD families to identify goals and potential referral pathways
  • With family's consent, facilitate warm referrals to appropriate services for meaningful engagement and wrap around support.
  • Empower families to access culturally relevant information, support and education around gaps in knowledge associated with social determinants of health
  • Foster partnerships and cross-cultural knowledge exchange between government, business, not-for-profit organisation and CALD community in Logan to improve outcomes for families
  • Reporting and administration, including the accurate keeping of records and appropriate documentation, data collection
  • Actively learn and understand community gaps in knowledge of services, supports and systemic processes with a focus on health and wellbeing across Ipswich and Logan.
  • Advocate for and contribute to strengthening culturally responsive practices across the sector to meet the needs of CALD families
  • Other duties as directed from time to time within your skills range and capacity.

About You

You're a compassionate and skilled professional with qualifications (or equivalent experience) in community or human services. You bring strong communication and listening skills, with the ability to understand client needs and connect vulnerable individuals to the right supports. You're confident working with culturally and linguistically diverse (CALD) communities and can engage in a culturally responsive way, speaking a community language is a plus

You thrive both independently and as part of a team, and you're comfortable collaborating with a range of internal and external stakeholders. Experience supporting refugees, asylum seekers, or vulnerable community members, along with familiarity using databases and Microsoft applications, will be highly regarded.
