The opportunity:
We are looking for a Speech Pathologist to join our supportive and professional multidisciplinary teams in a flexible part time capacity.
Responsibilities include but are not limited to;
- Provision of high-quality Speech Pathology services to all healthAbility clients from various funding sources.
- Understand the business rules for HACC, CHSP, CH, ICD, MBS, HCP and NDIS and achieve targets in all areas. This may include new future funding areas to the organisation.
- Provide relevant assessment and progress reports to the relevant funding partners in a timely manner.
- In collaboration with the internal In-Home Support Team, Support Coordination and Home Care Packages Team, identify, organise, plan, implement and evaluate evidence-based services.
- Work closely with Support Coordinators, LAC/NDIA Planners or Care Managers etc to identify and appropriately manage participant needs.
- Actively involve clients and/or carers in setting goals and making decisions about their care.
- Participation in continuous improvement and planning processes.
- Ensure all clinical service delivery records and funding applications meet all requirements for funding bodies.
- Contribute to the development and innovation of integrated multidisciplinary service delivery models.
- Participate in clinical supervision and ongoing professional development.
Our ideal candidate:
- At least 1 year of clinical experience in relevant areas
- Demonstrated ability to provide customers with an exceptional service experience
- Ability to work with a high level of independence and autonomy and accountability and able to escalate to senior staff for support when required
- Ability to develop, maintain effective client centred therapeutic relationships with clients, families and carers
- Demonstrated experience in providing services in both individual and group-based settings
- Demonstrated ability to work effectively in a team and to foster teamwork
- High developed interpersonal skills with sound communication and literacy skills
- Motivated and willing to thrive
- Demonstrated ability to manage workload and meet specified targets
- Victorian Drivers License
