The Older People's Mental Health (OPMH) Pathways to Community Living Initiative (PCLI) Mental Health Clinician will work as part of the Stage 1 Pathways to Community Living Initiative (PCLI) multidisciplinary team to provide high quality, evidenced informed assessments and clinical care to consumers to ensure their safe transition from hospital to community-based living.
This work is carried out in close collaboration and partnership with the consumer and their carer(s), the Inpatient multidisciplinary treating team Community Mental Health Teams, General Practitioners, aged care services, residential aged care facilities and Non-Government Organisations.

What you'll be doing
The Pathways to Community Living Initiative [PCLI] is a coordinated state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for more than twelve months to re-establish their lives in the community. The Initiative was established as part of the Government's commitment to strengthen mental health care in NSW by developing effective community-based residential care and support options for people experiencing long stays in mental health inpatient units.
The OPMHS PCLI consumer cohort requires specific and specialist assessment and interventions associated with the complexity of their clinical presentations including the presence of co-morbidities such as physical health problems, drug and alcohol use / abuse and issues of ageing including Dementia and the Behavioural &Psychological Symptoms of Dementia.
The primary function of the OPMHS Mental Health Professional is to work as part of the Stage 1 Pathways to PCLI multidisciplinary team to provide high quality, evidenced informed assessment and clinical care to consumers who are aged or have ageing issues to progress the safe transition into care in a community setting based on the individual consumer's strengths, needs and wishes. This work is carried out in close collaboration with the consumer &their carer/s, the Inpatient Mental Health Teams, Community Mental Health Teams, General Practitioners, Aged Care Services, other medical professionals and Non-Government Organisations (where applicable).
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Current Australian Health Practitioner Regulation Agency (APRHA) registration as a Registered Nurse, Clinical Psychologist or Occupational Therapist or eligibility for membership to the AASW. Registered Nurses must have relevant post registration qualifications and at least 3 years' experience working in Older People's mental health or such other qualifications or relevant equivalent experience. Allied Health Professionals must have experience as level 2.
- Demonstrated skills in conducting comprehensive mental health assessments and ability to develop and implement care plans and discharge plans for older people with severe persistent mental illness and complex issues.
- Demonstrated capacity to comprehensively assess and manage risk associated with suicide, deliberate self-harm , reputational risk and risk associated with violence, exploitation and abuse.
- Demonstrated experience working in Community Mental Health services, clinical expertise, clinical leadership and the ability to work effectively and collaboratively both independently and as part of the multidisciplinary team, key internal and external stakeholders.
- Current NSW Drivers License.
