The Role:
Be the operational engine behind safe, consistent participant supports—keeping cases moving, risks controlled, and documentation airtight while you grow toward full Care Coordinator.
What you'll do:
- Onboard & Plan: Set up consents, goals, risks, and files (Flow Logic or similar); schedule reviews and check-ins.
- Translate Plans → Rosters: Spot gaps (skills, availability, transport) and drive fixes with Rostering/Recruitment.
- Document to Standard: Same-day, objective progress notes; maintain risk, behaviour support, health and safety plans.
- Safeguard & Escalate: Support incident management, corrective actions, and continuous improvement.
- Coach & Coordinate: Provide guidance to Support Workers; facilitate competency checks and mandatory training.
- Stakeholder Liaison: Engage families, plan managers, therapists, and internal teams with clear, respectful updates.
- Budget Stewardship: Track utilisation vs funding; flag overs/underspend and change-of-circumstance early.

What you'll bring
- Experience in supporting and working with care coordinators, client services, disability/community care, or case management.
- Strong NDIS literacy (plans, budgets, service agreements, provider duties).
- Excellent communication skills: respond effectively and empathetically to participants/families while upholding professional boundaries (trauma-informed practice).
- High standards in documentation, time management, and follow-through.
- Confident with CRM/rostering systems, MS/Google Suite.
- Current WWCC, NDIS Worker Screening Check, Police Check, Driver's Licence (own vehicle).
- Qualifications in Disability, Allied Health, Social Sciences, or Nursing are a plus.
