Position Overview:

We are seeking an experienced Full Time or Part Time NDIS Support Coordinator to join our friendly office environment in Pakenham. The successful candidate will work directly with NDIS participants, their families, and other stakeholders to implement, monitor, and review NDIS plans, ensuring that all supports, both funded and informal, are effectively coordinated to enhance participants' quality of life.

Key Responsibilities:

  • Work collaboratively with NDIS participants, families, and other stakeholders.
  • Implement, monitor, and review NDIS plans.
  • Coordinate all supports in the plan, including informal, mainstream, community, and funded supports.
  • Connect participants with both formal and informal support.
  • Maintain ethical, participant-focused practice to help participants achieve their goals.
  • Ensure timely and accurate record-keeping for accountability and service efficiency.

Skills and Experience:

  • Proven experience as an NDIS Support Coordinator is essential.
  • Excellent time management and organisational skills.
  • Ability to prioritise work and meet deadlines.
  • Strong understanding of the NDIS and its impact on the lives of people with disabilities.
  • Ability to assess and respond to individual needs effectively.
  • Strong communication skills, capable of building trust and rapport with diverse individuals.
  • Familiarity with local services, organisations, and community opportunities.
  • Computer Literate.
