About the role

My Playground Quality Care is seeking an experienced NDIS Support Coordinator to join our team in Melbourne, VIC. This is a part-time position that will play a vital role in ensuring our clients with disabilities receive the highest quality of care and support to achieve their personal goals.

What you'll be doing

  1. Develop and implement individualised support plans for clients, in line with their NDIS funding and goals
  2. Coordinate and schedule client appointments and services with a range of providers
  3. Advocate on behalf of clients to ensure they receive the support they are entitled to
  4. Monitor client progress and make adjustments to support plans as needed
  5. Build and maintain strong relationships with clients, their families, and service providers
  6. Maintain accurate and up-to-date client records and documentation
  7. Collaborate with the wider team to continuously improve our service delivery

What we're looking for

  1. Relevant tertiary qualification in a human services field (e.g. social work, disability services, community services)
  2. Tertiary qualification must be specifically in disability services or community services
  3. Minimum 2 years' experience as an NDIS Support Coordinator or similar role
  4. Excellent communication and interpersonal skills, with the ability to empathise and build trust with clients
  5. Strong organisational skills and attention to detail
  6. Thorough knowledge of the NDIS and disability services sector
  7. Ability to work independently and as part of a team
  8. Current driver's licence and access to a reliable vehicle
  9. Australian citizenship is required for this position
