About the Role

Coordinate access and support services for clients seeking assistance from ATSICHS Brisbane, helping them build connections and engage support services to enhance their independence and community inclusion. Assist prospective ATSICHS Brisbane clients with NDIS applications and appeals to improve acceptance rates for Aboriginal and Torres Strait Islander people living with a disability.

Duties

  • Assist potential participants in completing the Access Request Form and coordinate with health professionals for necessary documentation.
  • Support participants through the NDIS application process, including appeals.
  • Refer non-eligible clients to external service providers.
  • Maintain accurate and detailed case notes and participant records.
  • Report on NDIS team data and activities.
  • Work with program networks to identify and reach target participant groups.
  • Plan and assist with NDIS information and Awareness sessions.
  • Respond to NDIS-related queries and contribute to resource development.
  • Maintain consistency and accountability in quality management systems.
  • Stay updated on NDIS systems and practices.

Skills and Experiences:

COVID - 19 Requirements

  • Immunisation history showing you have received two doses of a Covid 19 vaccine is encouraged.

Required

  • Knowledge, understanding, and/or awareness of the NDIS space and the challenges participants may face.
  • Demonstrated ability to record accurate and consistent case notes.
  • Demonstrated ability to apply practice judgment when working with clients in building capacity to achieve goals.
  • Demonstrated ability to work with minimal direction and to effectively communicate and work collaboratively as part of a multi-disciplinary team.
  • Strong organisational skills and ability to work independently and/or as part of team.
  • Ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people.

Additional Requirements

  • National Police Certificate.
  • Current Working with Children's Card (Blue Card) or eligible to apply.
  • Current Disability Worker Screening Worker (Yellow Card) or eligible to apply.
  • Current Driver's Licence.
  • Evidence of Work Rights.
  • Travel between work sites on a regular basis.
  • Travel to work in the community on a regular basis.
