We're looking for an enthusiastic, motivated, and caring nurse who enjoys working closely with patients and families, and who thrives in a small, close-knit team.

About the Role

This is a diverse and rewarding role combining general nursing care with a strong focus on health promotion, chronic disease management, and health program development.

You'll work alongside our GPs to deliver high-quality, holistic care, while having the independence to plan and run your own health initiatives within the clinic.

Key Responsibilities

Health Promotion & Programs

  • Develop and coordinate health promotion activities such as flu clinics, wellness checks, vaccination drives, and lifestyle programs.
  • Create and manage health education initiatives (e.g. diabetes, heart health, women's and men's health, smoking cessation).
  • Support patients in improving health outcomes through education and follow-up.

Chronic Disease Management

  • Prepare and review GP Chronic Condition Management Plans (GPCCMP)
  • Conduct Health Assessments (e.g. 45–49, 75+, Aboriginal and Torres Strait Islander).
  • Work with patients to monitor progress and ensure coordinated, ongoing care.

Clinical & Procedural Support

Provide general nursing care including wound care, immunisations, ECGs, spirometry, and pathology collection.

Maintain a safe and clean clinical environment in line with infection control standards.

Assist doctors with minor procedures, such as:

  • Skin excisions
  • Iron infusions
  • Circumcision
  • Vasectomy
  • IUD and Implanon insertions/removals

Team Contribution

  • Be an active part of our small, supportive team.
  • Participate in quality improvement and accreditation activities.
  • Help identify opportunities to improve patient care and clinic processes.

About You

Essential:

  • Current registration with AHPRA as a Registered or Enrolled Nurse.
  • Experience in General Practice or Primary Health Care.
  • Experience with GP Management Plans and Health Assessments.
  • Immunisation accreditation
  • Strong communication, organisation, and problem-solving skills.
  • Confident working independently and collaboratively.
  • A genuine passion for patient-centred care and health promotion.

Desirable:

  • Familiarity with clinical software - Medical Director.
