ONCALL Group Australia is a leading provider in Disability and Community Support Services. Our vision is to be nationally recognised as the leading integrated community service provider, empowering people to live their best lives.

ONCALL is seeking a motivated and compassionate Support Coordinator to join our Queensland team on a full time basis. In this key role you will help our clients with coordinating NDIS plans, negotiating appropriate support, and accessing a range of mainstream services and community support.

What you'll be doing:

  • Helping NDIS participants to understand their funding.
  • Connecting participants with providers, services and community supports.
  • Setting up service agreements and monitoring service delivery.
  • Tracking service delivery hours and billing accordingly, ensuring that Support Coordination funding is managed effectively.
  • Strengthening and enhancing participants' abilities for greater independence.
  • Ensuring all required documentation and reporting is completed as required and in line NDIS Practice Standards.

What you will bring:

  • A sector relevant qualification, e.g. Bachelor of Social Work/ Psychology, Allied Health, Health Sciences or Community Services.
  • Proven experience in a role as a Support Coordinator.
  • A track record of supporting participants to access the right supports, to develop and achieve sustainable goals in line with their NDIS plan.
  • The expertise to build the capacity of clients and their networks to coordinate their own supports.
  • Your excellent assessment and planning skills that enhance outcomes for people with NDIS funded support.
  • Excellent communication, organisational and problem solving skills.
  • Relevant industry background checks
