For nearly 100 years, Legacy WA has provided practical, person-centred support that enhances mental health, financial stability, and social wellbeing.


The Role

We are seeking a compassionate and highly organised Case Management Coordinator to provide holistic, trauma-informed support to veterans' families.

This role will:

  • Conduct comprehensive assessments and develop individual support plans.
  • Provide direct case support, advocacy, and crisis intervention.
  • Facilitate warm referral pathways to external providers.
  • Collaborate with Legatees, service providers, and internal teams.
  • Contribute to the design, delivery, and evaluation of community service programs.
  • Coordinate access to respite, camp, and peer support programs.
  • Maintain accurate case records and reporting in line with compliance standards.

This position is office-based with regular travel to client homes, service providers, and community events.


Key Responsibilities

Case Management & Client Support

  • Assess needs and develop tailored support plans.
  • Deliver strengths-based, trauma-informed care.
  • Provide crisis response, advocacy, and referrals.
  • Ensure risk assessments, safety planning, and mandatory reporting where required.

Community Programming & Engagement

  • Support the implementation and evaluation of wellbeing programs.
  • Coordinate participation in camps, workshops, and social activities.
  • Enhance peer-led support with Legatees and volunteers.

Stakeholder Coordination

  • Build and maintain strong relationships with government, ex-service, and community organisations.
  • Liaise with health professionals, financial advisors, and social workers.

Compliance & Reporting

  • Maintain accurate case notes and data.
  • Contribute to program reviews, funding reports, and policy improvements.
  • Participate in professional development and training.

About You

Essential

  • Bachelor's degree in Social Work, Psychology, Human Services or related field.
  • 2–3 years' case management experience with vulnerable populations.
  • Strong knowledge of trauma-informed and person-centred practice.
  • Excellent communication, advocacy, and stakeholder engagement skills.
  • High-level organisational skills and ability to manage multiple cases.
  • Current National Police Clearance & Working with Children Check.

Desirable

  • Experience working with veteran families, mental health, or financial support programs.
  • Lived experience within the veteran community.
  • Experience working with First Nations communities.
  • Mental Health First Aid, ASIST and/or SafeTALK training.
  • Advocacy Training and Development Program (ATDP) accreditation or willingness to obtain.
