**Support Coordinator**

Anala is seeking a Support Coordinator to join the team.

Anala offers a high standard of supports for people living with complex care needs such as disabilities, mental health and trauma. Our programs and services are designed to ensure each and every participant is supported to reach their goals and live their best life.

**The role**

The Support Coordinator is responsible for supporting participants, their family and carers in identifying and connecting with appropriate providers of supports with a view to overcoming barriers, developing skills and achieving the objectives of their NDIS Plan.

**Duties**
- Provide Support Coordination to assist the participant to implement their NDIS plan by maximizing choice and control and engaging high quality service providers.
- Be a key point of contact for the participant for the NDIA and all services and support to ensure the full implementation of the participants NDIS plan.
- Establish and maintain clear lines of communication and consultation between participants, their guardians, families, and other service providers, particularly in relation to Participant individual planning and related activities.
- Ensure participant goals and level of attainment is monitored, adjusted and recorded.
- Select preferred options or providers based on the chosen selection criteria.
- Arrange any assessments required to determine the nature and type of funding required.
- Monitor and adjust participants budgets to ensure NDIS plan outcomes are optimised.

**Criteria**
- Minimum Cert IV Qualification in Community Services Sector/or similar
- Working with Children Check
- Current Driver’s Licence and Vehicle
- NDIS Worker Orientation Module and New Worker - Induction Module

**Job Type**: Part-time

**Salary**: $36.00 - $41.52 per hour

**Benefits**:

- Travel reimbursement

Schedule:

- Day shift
- Monday to Friday
- No weekends

**Education**:

- Certificate I - IV (preferred)

Licence/Certification:

- Driver Licence (required)
