Program Manager

Manage a new program that supports members of the community to engage with the Aged Care System including key communities like CALD, LGBTQIA+, Aboriginal and Torres Strait Islanders, and people who are likely to disengage with Aged Care due to past trauma. This will likely include people who have been homeless, experienced DV, or have ongoing mental health or AOD concerns.

- The office based in Chatswood with geographical scopes across Northern and Western Sydney.

**_The Role:_**
Work for a reputable NFP to deliver and manage the rollout of the new Care Finders Program.
- Lead the team to deliver quality case management services to key target groups excluded from Aged Care services in Northern and Western Sydney.
- Recruit, develop, supervise, and support a team of Intake Officers, Case Managers, and Team Leaders.
- Oversee and develop a community engagement strategy to ensure target groups are engaged with.
- Attend and participate in communities of practice stakeholders with the support of the Practice Team Leader.
- Be accountable for the team meeting the program KPIs and operational plans in line with guidance from the Executive.
- Ensure program reporting requirements are met.
**_About yourself:_**
- You have 3-5 years of management experience supporting case managers.
- You have case management experience working with one or more of the key target communities or groups; CALD, LGBTQIA+, Aboriginal and Torres Strait Islanders, homelessness, victims of DV, and AOD.
- You have knowledge of the aged care space.
**_Requirements for the role:_**
- Double vaccinated against COVID-19 OR valid medical exemption
- Able to conduct Police Check
