
Care Coordinator

**ABOUT THE ROLE**:
The Care Coordinator is an integral role in the functioning of the Mercy Health Home Care service through the provision of coordination and administration support to the Care Advisory teams.
- This role will assist in coordinating services to Mercy Health clients living in a home care setting. The client group is varied and includes but not limited to:
- Home Care Packages
- CHSP clients
- Brokered services
- DVA/VHC clients
- Private
- fee for service clients
- NDIS clients
- TAC clients
Reporting to the Senior Care Advisor, the Care Coordinator position will work closely with the Care Advisory and Service Coordination teams in the designated geographic region and will have key responsibility for meeting compliance requirements in relation to Aged Care Act 1997, Home Care Standards, consumer directed care principles and Mercy Health policy and procedures.
