As our team continues to grow, we’re excited to offer a fantastic opportunity for a **Support Coordinator/ Support Service Officer** to join us at our Head Office in Revesby.
**About the Role**:
As a Support Coordinator, you’ll play a vital role in empowering NDIS participants to achieve their goals. Reporting to the Case Manager, your responsibilities will include:
- Building strong, meaningful relationships with participants and their families.
- Supporting participants to understand their NDIS plans and navigate available supports.
- Facilitating and coordinating services to help participants achieve their goals.
- Identifying and overcoming risks or barriers with effective solutions.
- Coaching participants and their families to enhance resilience, capacity, and decision-making skills.
- Collaborating with service providers, allied health professionals, and other stakeholders.
- Developing recovery-oriented plans and enabling personal growth.
- Preparing plan review reports and assisting participants in managing their NDIS funds.
- Documenting progress and reporting effectively.
**What We’re Looking For**:
- **A minimum of 1 year of experience in a similar role** with strong knowledge of NDIS guidelines and practices (essential).
- Exceptional communication and interpersonal skills.
- Proven problem-solving abilities to tackle complex challenges.
- A valid driver’s licence and access to a reliable vehicle.
- A genuine passion for psychosocial recovery and empowering others.

Pay: $30.00 - $35.00 per hour
Expected hours: No more than 20 per week
