**Company Background**: Cumberland Care Service has been providing support to those in need since 1995. We provide a range of services including Aged Care and Disability services to participants with approved plans under the NDIS. We take the time to get to know our participants personally which enables us to provide individualised services enabling participants to have choice and control over the services that they receive.
**Cumberland Care Service **are a seeking qualified, reliable and passionate Care Coordinator. You will need to be friendly, positive, and determined to make a difference, demonstrating client-focused behaviour, showing empathy, and treating others with dignity and respect. Building relationships with our clients whilst being honest, helpful, and dependable, making you a great asset to our team and clients.
**Duties include**:
- Assisting with handling incoming telephone and internet enquiries.
- Scheduling services for our clients by rostering Care workers dependent on their qualifications and availability. Creating master rosters as well as managing daily changes to existing rosters.
- Assisting supervisor with Recruiting, training, and managing the performance of the care team.
- Maintaining client and care worker compliance and documentation according to government standards.
- Ensure all documentation is accurately and comprehensively completed in Cumberland Care Service databases and software programs in accordance with organisation policies and procedures and legislative frameworks.
**Preferred Skills**
- At least 1 year experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS support coordinator
- Solid knowledge in the Disability and/or Aged Care sector(s)
- The ability to operate efficiently and deliver excellent service
- Time management, organisational skills & ability to manage competing priorities
- Relationship development skills to support diverse clients to achieve their goals and objectives
- Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS)
**Salary**: $55,000.00 - $60,000.00 per year
Schedule:
- Day shift
- Monday to Friday
