As the Support Coordinator, you will oversee and coordinates the operational running and program delivery at Horizon House accommodation locations, including associated transition placements.

The Position- Responsible for the day-to-day operational coordination and leadership of Horizon House accommodation locations and support programs.- Assist with the recruitment of caregivers and facilitate an induction to the role and program.- Facilitation of regular team meetings including case discussions and service coordination. Provide line management of team members including regular supervision, performance development and individual de-briefing as required.- Support a trauma informed approach and embed with all caregivers in this area.- Work in collaboration with the Practice Lead to prepare and assess new referrals for presentation at Assessment Panel meeting.- Responsible for case management and the development and review of client plans with each program participant, assisting and supporting the transition and case management of participants moving in, out and between the different service tiers.- Develop strong networks within the local community and actively promote the service.- Support the State Manager to monitor and maintain resources within the allocated budget; manage the day-to-day running, general maintenance, reporting requirements and capital expenditure of the accommodation services within the designated area.- Create a harmonious and productive working environment by setting and adhering to high personal integrity and performance standards at all times.- Maintain positive and clear professional relationships with colleagues and young people in the program.- Ability to treat young peoples’ concerns with respect, tact, sensitivity, whilst being aware of the limits that are required by confidentiality and the necessary boundaries that govern the youth/youth worker relationship.- Participate in the out of hour’s roster as On Call Coordinator.- Be responsible for maintaining current First Aid and CPR Certification.

To succeed tertiary qualification from a recognised University and/or TAFE College in Youth Work, Social Work or Psychology and a minimum of 5 years’ experience working with young people in a residential care setting. You will possess experience in program coordination and leadership of a team to successfully support program delivery.

Ability to travel and an unrestricted state driver’s licence will be essential to this role.

We can offer you- Salary: $94,159 to $98,425 (pro rata if part time) + superannuation- Permanent full time position, working 76 hours per fortnight- Salary packaging up to $18,550 on a range of benefits such as mortgage, rent, meal entertainment, holiday accommodation or other everyday living expenses as well as options to salary package benefits above the FBT cap on items such as:
