We are an established Disability Service Provider company provided our services to the NDIS participant age group from 7 to 65 years across the Victoria. We believe that people with disability, irrespective of age, gender, cultural or linguistic background, religious beliefs, geographic location, sexuality, or the nature, origin, or degree of disability have a right to participate in the community and are entitled to a decent standard of living.

My Core Support are passionate, innovative, and fearless in the promotion and defence of the rights and interests of people with disability. We provide support to the individuals who have - ABI, ASD, Intellectual, Physical, Mental, Cognitive, Neurological, multiple, and complex needs and disability. This role is accountable for the delivery and continuous improvement of high-quality person-centered service to clients, aligned to My Core Support’s Mission, Vision and Values.

**Position Overview**:

- Look after the physical, emotional, cultural, and social needs of the participants using a person-centered approach
- Observe and promote the participants’ choice, independence, dignity, privacy, fulfillment, and other rights
- Create and maintain good professional relationships with participants, their family, friends, and other stakeholders
- Actively support other Support Workers
- Adhere to all regulatory and statutory obligations and My Core Support Pty Ltd policies, procedures, and guidelines
- Actively market My Core Support Pty Ltd and promote a positive, personal and professional profile, ensuring the good reputation of My Core Support Pty Ltd at all times

**Roles and Responsibilities**:

- Care provision_
- Complex Behavior handling experience “Intellectual Disability, Autism spectrum, Dementia and Mental health” repetitive behavior, Depression, Bipolar disorder & borderline personality disorder”
- Epilepsy clients
- Know and understand the care and support of the participant
- Undertake the tasks detailed in the participants’ Care Plan using a person-centered approach and in the least intrusive way
- Encourage the independence and motivation of the participants and not foster dependent behavior
- Provide input into the care and participants plans of participants by regularly feeding back to the Disability Services Coordinator/Support Coordinator
- Assist participants to wash, bath and shower
- Assist participants to dress and undress
- Assist participants to look after their skin, teeth, hair, and nails
- Assist participants with toileting, continence management and personal hygiene
- Prepare food and drink for the participants, being aware of the participants’ choice, likes/dislikes, nutritional needs and cultural requirements
- Provide light general household domestic duties, including housework and laundry, as detailed in the Care Plan or instructed by NDIS Services Coordinator
- Use manual handling equipment safely and correctly
- Take responsibility for the safe handling of property and equipment belonging to the participants
- Maintain good communication and develop effective working relationships with participants
- Provide companionship to the participant, actively talking and listening to them about their interests
- Help the participants to maintain contact with their family and friends
- Accompany the participants on trips into the community
- Ensure as safe as possible the living environment for the participants, whilst respecting the participants’ choice and rights
- Operational Duties_
- Dress appropriately and use personal protective equipment provided by My Core Support Pty Ltd
- Seek out best practice and look at innovative ways to improve the quality and efficiency of service delivery

Maintain effective communication and provide regular written participants progress reports to NDIS Coordinator.
- Attend and participate in regular Support Worker meetings and any other relevant meetings
- Attend in house and external training pertinent to the role of Support Worker
- Ensure completed fortnightly timesheets are submitted to the Administration/Rostering on time
- Observe all work, health & safety rules and take reasonable care to promote work health and safety of self and others and raise any concerns to the NDIS Coordinator
- Aim to ensure everyone has equal treatment and equal access to services and employment
- Any other duties requested by Management, which are within the scope of the position
