**About the Role**
Are you passionate about supporting older Australians and making a genuine difference in their lives? In this vital role, you’ll work closely with patients, families, and healthcare professionals to ensure smooth transitions from hospital to residential aged care or safe community living. Reporting to the TCP Service Manager, you will provide a task-oriented welfare service focused on meeting the practical needs of patients, helping them access essential benefits and services to support their hospital stay, discharge, and long-term care.
As the key contact for Placement Services, you’ll build and maintain strong relationships with aged care facilities, patients, and their families. Your knowledge of aged care options will be instrumental in guiding informed decisions and coordinating appropriate placements. This is a rewarding opportunity to collaborate with multidisciplinary teams, advocate for vulnerable patients, and contribute meaningfully to their wellbeing and quality of life.

- **Strong interpersonal and communication skills**, with the ability to advocate for patients, manage sensitive situations, and work effectively with individuals from diverse backgrounds.
- **Proven experience in aged care or hospital settings**, including navigating placement options and working directly with patients, families, and support services.
- **Well-developed organisational and problem-solving abilities**, including time management, prioritisation, negotiation, and report writing.
- **Sound knowledge of aged care services and support systems**, as well as an understanding of confidentiality, patient welfare, and the impacts of ageing, disability, and grief.
