Rehabilitation, Aged and Palliative Care Services at the Southern Adelaide Local Health Network (SALHN) provides team based care for older people, people who are living with a disability, and people with a life-limiting illness.

**About the role**:
Reporting to the Director of Social Work - RAP, as the Welfare/Placement Officer, this client facing role facilitates the assessment, listing and placement of those admitted under the Division of Rehabilitation, Aged Care and Palliative Care (Division of RAP) who have been identified by clinical staff as being inappropriate for discharge to their previous residence. Duties will include:

- Maintain statistical and management information for planning, monitoring and reporting purposes, with emphasis on key performance indicators for the Aged Care Assessment Team and placement process, and on patient transfers to Residential Aged Care Facilities and Transitional Care places,
- Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.

**About you**:

- Ability to relate to a wide range of people, including patients, families, clerical staff, clinical staff, Aged Care Assessment Teams and Directors, Supervisors and Managers of Residential Aged Care Facilities and Supported Residential Accommodation facilities,
- Good oral and written communications skills, ability to negotiate, and to manage conflict,
