**Southern Adelaide Local Health Network is seeking a passionate, and caring Residential Care Placement Officer to join our Rehabilitation, Aged and Palliative Care Team**

**About the role**:
Reporting to the Director of Social Work RAP, as the Placement Officer you will assist the assessment, listing and placement of patients from hospital, liaise with stakeholders and maintain record systems. Duties for the role will include:

- Liaise with Residential Aged Care facilities and other aged care and community service and accommodation providers to facilitate access to these facilities and services by patients of the Division of RAP
- Monitor vacancies in Residential Aged Care Facilities, Supported Residential Accommodation Facilities and community programs
- Provide, in consultation with Division of RAP Social Workers, advice and assistance to patients and families of patients regarding assessment processes for residential aged care facilities and on the availability and access to these facilities

**About you**:

- Exceptional interpersonal skills with experience supporting members of the community
- Ability to analyse statistical data and develop options and make recommendations on this basis
- Experience in using a range of software packages, e.g. excel spreadsheets and database packages
- Ability to work under pressure, to co-ordinate and prioritise a number of tasks simultaneously to meet deadlines
- Good oral and written communications skills, ability to negotiate, and to manage conflict
