**About the Role**:
The Northern Adelaide Local Health Network is seeking an exceptional individual to join our Aged Care, Rehabilitation and Palliative Care division as a Placement Officer.

The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.

As a Placement Officer, you will play a crucial role in ensuring the efficient placement of patients within our healthcare system. Your responsibilities will include liaising with various healthcare providers, coordinating patient transfers and managing admission and discharge processes. You will work closely with multidisciplinary teams to ensure seamless transitions and continuity of care for our patients.

To excel in this role, you must possess exceptional communication and interpersonal skills, enabling you to navigate complex situations with empathy and professionalism. Strong organisational abilities and attention to detail are essential, as you will be responsible for maintaining accurate patient records and adhering to strict protocols.
