The Early Supported Discharge - Rehabilitation Service (ESD) is a bed substitution program embedded within the Community Rehabilitation service. ESD accelerates the discharge of stroke and other neurological patients admitted to hospital. The program provides intensive, multidisciplinary rehabilitation in the home and community setting immediately following discharge.

The Coordinator is integral to ensuring that the ESD team offers high quality, effective multidisciplinary and interdisciplinary rehabilitation. The Coordinator will work collaboratively with the broader healthcare team to deliver seamless care to the ESD clients. The Coordinator will liaise directly with inpatient staff, patients and their families to facilitate discharge and coordinate comprehensive rehabilitation programs.

The ESD Coordinator will provide clinical leadership to the care team, supporting complex problem solving and skill development. They will also provide strategic leadership to ensure that the service meets KPIs and operational requirements.
