Supporting Patient Care: Casual Social Worker Role

Casual Social Worker Grade 2-1 role involves providing support to the existing social work team across St Vincent's Private Hospital Melbourne campuses.
The jobholder is part of each ward's team, working with Nursing, Discharge Coordinators, Medical and Allied Health staff to facilitate effective patient care and discharge planning using community resources.
Key Responsibilities- Deliver medical-focused social work in a hospital inpatient setting, enhancing patient outcomes and experiences.
- Collaborate with multidisciplinary teams to address complex social needs, ensuring seamless patient transitions.
- Participate in the Aged Care Assessment Service (ACAS) assessment process, managing related administrative tasks efficiently.
- Develop and implement discharge plans tailored to individual patients' assessments, including engagement with carers and families.
- Coordinate community resources to ensure suitable discharge options are accessible and convenient for patients.
- Identify patients with complex social issues early and contribute to Multi-Disciplinary Team meetings as required, fostering a culture of collaboration and best practice.
