Effective Care Coordination Specialist

This role involves providing exceptional care coordination services to clients. The Case Manager will work closely with healthcare professionals and other stakeholders to ensure seamless patient transitions and optimal outcomes.
Key Responsibilities:
1. Participate in quality improvement initiatives to drive continuous excellence.
2. Conduct post-discharge follow-ups to assess client progress and address concerns.
3. Generate reports on key performance indicators, client outcomes, and relevant metrics.
4. Analyze data to identify trends, areas for improvement, and opportunities for optimization.
5. Manage referrals from healthcare professionals or agencies, ensuring timely communication and coordination.
6. Maintain accurate and up-to-date client records, including care plans, assessments, and communication logs.
7. Ensure compliance with privacy regulations and organizational policies.
8. Collaborate with multidisciplinary teams to optimize patient care.
9. Communicate regularly with clients and their families to assess satisfaction and address concerns.
10. Coordinate with care providers to ensure they are well-informed about clients' needs and preferences.
Requirements:
1. Bachelor's degree in Nursing (or equivalent experience).
2. Previous experience in care coordination or a related role in the healthcare industry.
3. Strong organizational, communication, and interpersonal skills.
4. Knowledge of home care regulations and industry best practices.
5. Ability to handle and resolve conflicts effectively.
6. Proficient in computer applications, including Microsoft Office and care management software.
7. CPR and First Aid certification.
8. Passion for delivering outstanding care to clients.
Preferred Skills:
1. Clinical expertise in direct client care and safety management.
2. Ability to develop, implement, and evaluate organizational and client-focused documentation.
3. Good understanding of government funding environments for aged care and hospital discharge processes.
4. Effective communication and liaison skills with hospitals and health professionals.
5. Ability to multitask and manage time effectively.
6. Excellent verbal and written communication skills.
7. Intermediate to advanced computer skills, including CRM systems.
