**Who are we?**

Wintringham is a not-for-profit organisation providing an extensive range of aged care, housing and related support services to men and women aged 50-plus who have been homeless or at risk of homelessness. We employ over 900 staff and operate from multiple offices across Melbourne and regional Victoria.

Wintringham’s Supportive Housing Project - the Audrey Rainsford Building is comprised of 60 beautifully finished and brand new apartments centrally located in Carlton. The Supportive Housing multi-disciplinary team consists of a Coordinator, Case Workers, Recreation staff and Tenancy staff, who are in place to support a staged approach to independent living and enhance the community engagement of its residents.

**Your new role**

This is an exciting opportunity for a professional experienced with Intensive Case management eager to demonstrate their exceptional leadership skills within our brand new Supportive Housing Project - the Audrey Rainsford Building. In addition to managing a caseload of residents with complex needs, this role will provide leadership, guidance and support to a team of Housing & Homelessness Support Supportive Housing Case Workers. As a point of escalation, you will manage issues and incidents on-site and ensure effective communication with key stakeholders and the Site Coordinator. This role will work collaboratively residents to develop individualized case plans through identifying their strengths, as well as their support needs.

**Your responsibilities and duties will also include**:

- Provide key leadership and practice support that guides and develops the program through a range of innovative and creative plans and strategies.
- Effectively supervise a team with mixed caseloads of clients requiring low to medium intensity support.
- Document and monitor income and expenditure against budget expectations including client brokerage funds.
- Actively participate in team meetings, supervision, reflective practice, annual performances appraisals and practice development across the portfolio.
- Work collaboratively with residents to create a meaningful life, assisting them to develop self-management skills across mental and physical wellbeing, personal safety, and crisis management and harm minimization
- Work with residents by linking and referring them into clinical services as well as social and recreational activities.
- Utilise your knowledge of harm reduction and brief intervention, trauma informed practice, conflict resolution, MARAM and Housing First principals to manage a case load of clients.
- Ensure residents obtain access to Commonwealth Home Support Program (CHSP), Home Care Packages and NDIS services by providing referrals and case management services.
- Responding to resident feedback and complaints in line with Wintringham policy and procedures.
- Utilising relationships within sectors to assist access into other services and systems.

- A tertiary qualification in Social Work, Welfare, Community Services or equivalent.
- Experience in a senior practitioner role with specific training and/or experience in: AOD Mental Health and Family Violence or Homelessness services.
- An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
- Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
- Well-developed communications skills and the ability to with engage people from varying backgrounds.
- First Aid Certificate.
