Home Care Package Coordinator

We have a position available for a **Home Care Package Coordinator.** You will have a minimum of 1 years home care package coordination experience and genuinely enjoy making a difference to older peoples lives. Building good relationships with clients, their families and other health professionals, you will be responsible for the coordination, care planning and monitoring of Home Care Package clients (Levels 1 to 4) to ensure a high quality service is provided. As a team player, you will enjoy working as part of the care team comprising HCP Coordinators, CHSP Coordinators, Registered Nurses and Allied Health. This is a full time position, based in the Brisbane CBD with company cars available for in-home client visits. All Aged Care has been providing quality care for 23 years. We are a multi-cultural organisation priding ourselves on delivering services which exceed client's expectations to empower clients to remain living independently in their own homes. To be successful in this role you will be self-motivated with a "can do" attitude and have: - Minimum 1 year Home Care Package Coordinator experience AND a care related qualification such as Case Management skill set or Diploma of Community Services - A solid understanding of the Home Care Package program including care planning, budgets and compliance requirements - Good communication, problem solving and organisational skills - Demonstrated ability to prioritise and multi-task - Good computer skills including with care databases, Outlook, MS Word and Excel - An ability to speak another language is highly regarded You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza. **Salary**: $70,000.00 - $75,000.00 per year **Benefits**: - Salary packaging Schedule: - Monday to Friday **Education**: - Certificate I - IV (preferred) **Experience**: - CHSP or HCP Coordinator: 1 year (required) Work Authorisation: - Australia (required) Work Location: In person
