We are seeking someone who has demonstrated experience as either a **CHSP Coordinator or Home Care Package Coordinator** who genuinely enjoys providing case management services for people over the age age of 65 years.

You will be responsible for the coordination, care planning and monitoring of CHSP clients to ensure a quality service is provided. You will enjoy building good relationships with clients, their families and other health professionals. You will work closely with our other CHSP and HCP Coordinators and Clinical team. This is a full time position, **based in the Brisbane CBD.**. A car is available for in-home visits.

All Aged Care has been operating for 22 years as a quality approved provider of home care services with Head Office located in the Brisbane CBD close to public transport. We are a multi-cultural organisation who pride ourselves on delivering services which exceed client's expectations which enable clients to remain living independently in their own homes.

To be successful in this role you will be self-motivated with a "can do" attitude and have:

- A minimum of 1 years experience as either a CHSP Coordinator or HCP Coordinator
- Diploma of Community Services or equivalent qualification
- Current First Aid and CPR
- Demonstrated experience in CHSP or HCP case management
- Strong organisational skills including the ability to problem solve, prioritise and multi-task
- A good team player with excellent communication and interpersonal skills
- Good computer skills including with care databases, word, excel, outlook
- Self-motivated with a "can do" attitude

You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza.

**Salary**: $70,000.00 - $75,000.00 per year
