Are you an organised, compassionate professional who thrives on making a difference in the lives of others? Right at Home Brisbane West is seeking a **Full-Time Care Coordinator (12 month contract with the possibility of ongoing)** to join our dedicated home care team, supporting clients across aged care and disability services.

**About the Role**

As a Care Coordinator, you’ll play a vital role in connecting our clients, their families, and our team of care professionals. Your focus will be on ensuring every client receives high-quality, person-centred support tailored to their individual needs.

**Key Responsibilities**
- Coordinate and schedule in-home care services
- Match clients with suitable support workers based on skills, preferences, and compatibility
- Maintain regular communication with clients, families, and care staff
- Monitor and update client care plans to reflect changing needs
- Ensure compliance with NDIS and Aged Care Quality Standards
- Deliver exceptional customer service and maintain accurate records

**What We’re Looking For**
- Minimum 2 years' experience as a Care Co-Ordinator
- Strong understanding of NDIS and Aged Care Quality Standards (preferred)
- Excellent organisational, time management, and communication skills
- Compassionate and client-focused mindset
- Proficiency in scheduling software and general IT systems
- Relevant qualifications:

- **Certificate III or IV in Individual Support (Ageing, Disability)** or
- **Diploma in Community Services**

**Qualifications**:
Certificate III Individual Support/ Diploma of Community Services or equivalent.

2 years experience in the health industry, preferably in care management.
