About the Health Kids Club
The Healthy Kids Club service provides care to children and young people who have overweight, obesity, and/or Type 2 Diabetes Mellitus. The community-based clinic services the multicultural Logan population to improve health outcomes of these families.
The Healthy Kids Club provides culturally responsive, family centred care via consultations, home visits, school visits, telehealth, and group sessions. The team aims to empower patients improve their health, achieve their goals through education, skill development, and holistic behaviour change. The population is complex requiring careful consideration when planning interventions, as well as maintaining continuum of care across other services
Role Details
Manage an outpatient paediatric overweight, obesity and Type 2 Diabetes Mellitus clinical caseload to deliver high quality level nutrition and dietetic services.
Identify and contribute to the development of evidenced based clinical practices, procedures and protocols that support the continuum of care.
Participate in state-wide education and outreach services.
Work cooperatively and effectively within a team and with patients / clients.
Communicate with patients and their families, other health professionals, supervisors, assistants, students and researchers across the community, hospital, home, school, and other settings across the state, as relevant to your service area.
Accurately and concisely record all patient interaction as well as workload measurement statistics, to facilitate the analysis of service trends.
Provide clinical leadership including caseload management, triaging and case formulation.
Provide student education and clinical practice supervision for less experienced practitioners, as well as direction to assistant, support staff and work experience students, under guidance of senior clinical practitioners.
Actively participate in the dietetic and clinical environments ensuring that dietetic services are integrated, efficient and equitable for the community.
Actively participate in the identification, development, and implementation of quality improvement activities for the delivery of dietetic and allied health services.
Develop knowledge and expertise through active learning within a team and professional development activities.
