The Salvation Army Youth Services offer an integrated suite of programs engaging with young people across Australia. Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.

We are currently seeking a values-driven Case Manager to provide case management to young people aged between 12-24 within the South Eastern Sydney Health District at Oasis Sydney - Drug and Alcohol Support Program (DAYS). The Case Manager will provide early crisis and long term interventions through a flexible case management model underpinned by trauma-informed care alongside team members who will also provide case management, youth counselling and group programs.

You will successfully
- Provide case management and care coordination for Young People (12-24) for up to 6 months
- Provide accessible information to clients, family, and other service providers about the service
- Assess clients with a range of co-existing physical health, mental health, and social conditions
- Develop effective and shared care relationships with other health and social service providers
- Establish the scope of services and/or resource supports for clients that may be accessed with brokerage

You will have
- Minimum Tertiary Qualifications in Social Work or similar discipline, preferably Bachelor’s Degree or Higher (other relevant qualifications/experience within Community Services, Social Work or similar discipline will be considered)
- Previous experience in case work or similar role, providing case management to vulnerable young people from diverse backgrounds
- Valid Employee NSW Working With Children’s Check
- Current NSW Driver’s Licence
- Proof of COVID-19 Vaccination or Exemption
- Eligibility for registration with peak body (where applicable)
- A willingness to undergo a National Police check
