About the Role
We’re seeking a motivated and experienced Support Coordinator / Specialist Support Coordinator to join our growing team.
This is a hybrid contractor role, offering flexibility and autonomy while allowing you to make a genuine difference in the lives of people with disability and complex needs. You’ll provide a combination of Level 2 Support Coordination and Level 3 Specialist Support Coordination services, working closely with participants, families, carers, and providers to achieve meaningful goals.
Your caseload will build gradually, ensuring you have the time and support to deliver exceptional outcomes.
Key Responsibilities
- Deliver a mix of Support Coordination (COS Level 2) and Specialist Support Coordination (COS Level 3) services.
- Manage a gradually increasing caseload of participants with varying levels of support needs.
- Coordinate supports and services for participants, including those with complex psychosocial, behavioural, or medical needs.
- Develop, implement, and review individualised support and service plans.
- Advocate for participants’ rights, preferences, and goals.
- Liaise with families, carers, service providers, and stakeholders to ensure a collaborative approach.
- Monitor risks, participant progress, and outcomes, ensuring high-quality service delivery.
- Maintain up-to-date case notes, reports, and compliance documentation aligned with NDIS requirements.
- Engage in supervision, team meetings, and ongoing professional development.
Qualifications & Requirements
- Bachelor’s degree in Social Work, Psychology, Allied Health, or a related field (highly desirable).
- Relevant governing body registration (if applicable).
- Minimum 6 months experience in Support Coordination or Specialist Support Coordination.
- Strong knowledge of the NDIS framework, complex needs, and high-risk case management.
- Demonstrated ability to work autonomously and manage caseloads effectively.
- Exceptional written, verbal, and interpersonal communication skills.
- Current National Police Check.
- Working With Children Check.
- NDIS Worker Orientation Module completion.
- First Aid & CPR (preferred).
- ABN and professional indemnity/public liability insurance (required for contractor role).
