We are seeking a motivated and compassionate Support Coordinator to join our team and help us coordinate client NDIS plans, negotiate appropriate support, and access a range of mainstream services and community support.

**We will value**:

- A sector relevant qualification, e.g. Bachelor of Social Work/ Psychology, Allied Health, Health Sciences or Community Services.
- A track record of supporting participants to access the right supports, to develop and achieve sustainable goals in line with their NDIS plan.
- The expertise to build the capacity of clients and their networks to coordinate their own supports.
- Your excellent assessment and planning skills that enhance outcomes for people with NDIS funded support.
