The role of **Team Leader** **(South/Logan) **with Lifest HealthCare (a NDIS Registered provider) will be working in with participants with varying diagnosis.

The purpose of the role is to facilitate homecare and community inclusion for participants, promote independence, decision making and personal choices while focusing on participants needs and goals.

The role of Team Leader is also responsible for managing a team of dedicated Disability Support Workers as well as administration duties relating to rostering, managing appointments for participants, report writing and attending regular meetings.

**Key duties include**:

- Work with and manage the teams responsible for caring participants in Brisbane South
- Work directly with participants who have varying diagnosis, including some with challenging behaviours and mental health concerns
- Support and encourage participants to partake in capacity building activities in the community and home care duties
- Complete all appropriate documents including shift notes, shift approvals, incident reports and monthly progress reports
- Participate in stakeholder meetings as required
- Train support staff to provide superior home care to participants
- Foster a culture of team inclusion based on strong communication, transparency, accountability and care in relation to supports and completion of compliance documents
- Report regularly to Management in relation to staff and participant wellbeing
- Lead by example, cover shifts during planned or unplanned leave if required
- Actively encourage participants to achieve their NDIS goals, assistant with medications and take participants to their appointments
- Host regular Team Meetings to update staff on various topics related to participant care and compliance

**Core Competencies**
- Sound knowledge of NDIS standards and practice and understanding the needs of people living with disabilities
- Effective interpersonal and communication skills
- Effective people manager with excellent time management skills

**Education & Experience**
- Cert III or IV in Disability or other relevant qualification
- Previous experience as a Team Leader is essential
- At least three years' experience in the Disability Sector
- Previous experience working with challenging behaviours/mental health concerns is highly desirable
- Tech Savvy with the ability to adapt to new technologies
- NDIS Worker Screening Card, Driver's License, First Aid, CPR and medications training etc

**Job Types**: Full-time, Casual

**Salary**: From $33.00 per hour
