**NDIS Team Leader/Area Coordinator - Brisbane South**

We are currently looking for an experienced Team Leader/Area Coordinator to join our team **full-time (Monday to Friday).** In this role, you will participate in and oversee the delivery of home and community disability supports for our participants and supervise a team of Support Workers across **the Southside of Brisbane (i.e. Logan, Redlands and Greenslopes).**This role involves a combination of working shifts directly with participants as well as relevant administration.
**Key responsibilities include**:
- **Work directly with participants on shift at least 60% of the time**on the Southside of Brisbane to provide community and home care assistance in accordance with individual care plans and NDIS goals, tasks will include assistance with personal cares, capacity building activities, monitoring and administering medications, organizing and attending medical appointments, promoting social and community inclusion and compliance reporting.
- Leadership & Supervision of a team of approximately 20 home care workers, including training and ongoing mentoring.
- Rostering at a minimum of 500+ hours per week - coordinate the fortnightly allocation of home care workers to participants based on participant care needs/hours and staff availability.
- Monitor the delivery of support services delivered to participants through review of timesheet submissions, shifts notes, behaviour and incident reports and any other relevant documentation.
- Respond promptly to participant feedback, concerns, or complaints, and take appropriate action to address issues and improve service quality.
- Ensure care plans and compulsory reports are maintained for each Participants and kept-up-to-date.
- Collaborate with external stakeholders including, support coordinators, health professionals, and other relevant parties.
- Communicate effectively with participants, their families, and relevant stakeholders regarding care updates, changes, or other relevant information.
- A Diploma or Degree in Community Services (Disability) or a Bachelor of Social Work or equivalent
- A minimum of three years' experience in the Disability sector - working directly with participants with mental health and complex behaviours
- Previous experience in the NDIS industry held in high regard
- Previous experience in managing large and diverse teams held in high regard
- Previous rostering experience to ensure sufficient shift coverage for 20+ participants
- Must be highly organised with excellent time management skills
- High-level of interpersonal and communication skills (written and spoken), with the ability to communicate effectively with staff at all levels of the organisation as well as external stakeholders
- Relevant valid compliance documents: NDIS Worker Screening Card, First Aid, CPR, Medications training etc
- Full working rights in Australia - **please note this role is not suitable for student visa holders**:
- Valid driver’s license with own vehicle with comprehensive car insurance (frequent driving is required)
- Must be available Monday to Friday during the day
- Proficient with technology

Pay: $80,000.00 - $86,000.00 per year
