Job Title: Disability Support Worker (Community Access & Participation)

Employment Type: Casual / Part-Time

**About Us**:Unique Care Services QLD is a trusted NDIS provider dedicated to empowering participants through high-quality support services. We specialize in community access and participation, ensuring our clients can achieve their goals while fostering inclusion and independence.

**About the Role**: We are seeking passionate and dedicated Support Workers to join our team. This role primarily focuses on providing community access and participation support to participants. Shifts generally run from 8:00 AM to 3:00 PM, Monday to Saturday.

**As a Support Worker, you will**:
Assist participants to engage in meaningful community activities, programs, and social events.

Support participants with accessing appointments, recreational activities, and day-to-day outings.

Promote independence, personal growth, and well-being.

Provide companionship, encouragement, and motivation to participants.

Collaborate with the team to implement care plans tailored to individual needs.

**Key Responsibilities**:
Support participants in achieving community participation goals.

Drive participants to and from activities (valid driver's license required).

Deliver support with dignity, respect, and a focus on empowerment.

Maintain clear communication with the team and accurate documentation.

Foster positive relationships with participants, families, and stakeholders.

**What We Are Looking For**:
A genuine passion for helping others and making a positive difference.

Flexibility to work between 8:00 AM and 3:00 PM, Monday to Saturday.

Previous experience in disability support, community access, or related roles (preferred but not essential).

NDIS Worker Screening Check or willingness to obtain one.

Current First Aid and CPR certification.

Valid Driver's License and access to a reliable, insured vehicle.

Excellent interpersonal and communication skills.

Ability to work independently and in a team environment.
