**Your new role**

**Engagement and support**
- Engage hospital functions such as social work, admissions, discharge planning, and clinic practice managers to encourage identification and referral of carers to Carer Gateway
- Promote awareness of Carer Gateway services and supports
- Identify, register and assess carers in hospital setting
- ** Marketing**:

- Contribute to the development of tailored engagement material and resources that support delivery of professional, meaningful and authentic engagement in a health setting
- Make formal and informal presentations to carers and organisations

**Reporting and administration**
- Ensure all activities are accurately collected, stored and reported against the performance criteria agreed in the strategy
- Ensure all data that can be stored in the organisational CRM is 100% accurate

**What you'll need to succeed**
- Demonstrated previous experience in a similar community engagement role
- Proven experience at efficiently planning and delivering activities that drive results
- Demonstrated engagement with individuals and community
- Lived experience is highly desirable
