Then you could be the perfect Integrated Care Services - Aboriginal Health Worker to join the professionals at Justice Health NSW. Firstly, applicants must be of Aboriginal descent through parentage, identification as being Aboriginal and accepted in the community as such. Aboriginality is a genuine occupational qualification and is authorised under Section 14(d) of the Anti-discrimination Act 1997. We are also looking for applicants that possess a Certificate III in Aboriginal and/or Torres Strait Islander Primary Health Care and/or equivalent study and/or work experience and a willingness to obtain the relevant qualification.

The Aboriginal Health Worker will work within the Integrated Care Service (ICS) based in the health centres to increase access to health treatment and prevention services for Aboriginal people, and have a strong focus on the prevention and management of chronic disease with an emphasis on improving the patient journey from custody to community. The Integrated Care Service aims to support and enhance the existing systems to provide an integrated model of care for adult patients in custody and on release into the community with chronic or complex health conditions. Furthermore, ICS is to monitor patients from entry at reception through custody and post release unto the community, ensuring they receive seamless, timely, appropriate and effective health care. The Aboriginal Health Worker will work within Justice Health and Forensic Mental Health Network (JH&FMHN) and Ministry of Health policies, procedures and guidelines as well as Corrective Services NSW or Juvenile Justice NSW rules and regulations as set out in the relevant legislation.

- Must be able to identify and implement health promotion activities within a secure environment for Aboriginal and/or Torres Strait Islander patients.
- Possess a current NSW drivers licence and the willingness to travel across the Network (as necessary).
- Possess the ability to work independently and as part of a team.
- Recent experience working with Aboriginal and/or Torres Strait Islander patients and their families and knowledge of their local cultural-specific communities and its health services.
- Demonstrated understanding and commitment to the values of the Justice Health & Forensic Mental Health Network.
