**Five days per week (31 hours)**
**Monday & Thursday (9.45am to 4.00pm)**
**Tuesday, Wednesday & Friday (9.00am to 4.30pm)**
**$27.50 to $28.30 p/hr range (depending on skills/experience)**

**About the role...**
We have a unique permanent part time opportunity for an experienced lifestyle coordinator to lead our positive, energetic and passionate team The role will develop and implement lifestyle programs to daily infuse our Elders with holistic wellbeing activities that are vital for their physical, emotional, cultural, mental and social wellbeing.
The Lifestyle Coordinator will demonstrate leadership by effectively mentoring and supporting the small friendly lifestyle team, including volunteers. The position will oversee the planning and implementing of quality person directed lifestyle programs that meet personal needs, individual care plan and accreditation requirements to support Elders maintain their independence and significantly enhance their quality of life. The position will collaborate daily with our multi-disciplinary team and different stakeholders. The successful incumbent will manage the program budget and will complete required documentation, policies and procedures, in accordance to accreditation standards. Experience in a similar role is a Must Some of the key responsibilities of the role include:
- Plan and coordinate a range of social and recreational activities and develop activities to maintain individual personal living skills and independence, including those living with dementia
- Ensure program documentation is completed in accordance to organisational and departmental guidelines
- Work collaboratively with elders, their representatives and the clinical care teams to identify the social, emotional and physical needs of individual elders to inform both the community program and individual lifestyle plans
- Assist in the development of organisational policy and programs as required
- Manage the program with budget parameters
- The needs of Elders with dementia and other specific requirements including cultural or religious differences are reflected in program delivery
- Documentation including social history, care plans, progress notes and minimum data requirements are maintained to the required standard
- Programs are evaluated in accordance with relevant guidelines
- Supervision and assistance is provided to other lifestyle staff and volunteers in the fulfilment of their duties.
**About you...**
You will have the ability to effectively manage time. You will be empathetic and model behaviour. You will be self-motivated, flexible, caring and have a patient attitude, with an ability to work in a professional and proficient manner.
**Qualifications**
- Certificate IV in Leisure and Health
- Certificate III Aged Care preferred
- Membership of a professional association related to lifestyle programs advantageous but not essential
**Skills**:
- Previous experience coordinating lifestyle programs in an aged care environment
- The ability to work in a culturally sensitive manner within an ethno-specific environment
- Excellent organisational and administrative skills strong capability to develop, manage and evaluate programs and activities
- Ability to work collaboratively and effectively with a broad range of stakeholders
- Strong understanding of the diverse needs of clients, including those living with dementia
- The ability to work in a culturally sensitive manner within an ethno-specific environment
