**CCDS is looking for**:

- A full-time support coordinator to join our Support Coordination team
- The role is being offered as a 12-month fixed-term position (has the potential to become ongoing as customer base grow)

**KEY RESPONSIBILITIES**:

- To assist our NDIS Participants to design, develop and implement their NDIS supports, connect our Participants to specialists and other supports through various services, as required or requested.
- To work directly with our Participant's and/or their carers/guardians to achieve their goals

**APPLIATION CRITERIA**:

- Minimum 12 month of experience within the NDIS environment
- Previous/sound experience or understanding of the role of support coordination.
- Strong connections and Knowledge of the services and activities within the southern region
- Ability to work independently and as part of a small team
- Competent in using Microsoft Office and efficient admin skills
- Experience in developing service agreements or other contractual arrangements
- A current Victorian Drivers licence, current Police check, working with children check, Australian residency status (right to work in Australia0 First Aid & CPR, and Proof of Vaccination
- Access to to comprehensively insured vehicle
- Experience working with internal and external stakeholders

ADVANTAGES: (Not Mandatory)
- Understanding and experience with mental health supports
- Tertiary qualifications in the human services field or equivalent

**Salary**: $65,000.00 - $75,000.00 per year

Schedule:

- 8 hour shift
- Monday to Friday

Supplementary Pay:

- Performance bonus
