JPlus Care Australia is a registered NDIS service provider, specializing in Support Coordination, Psychosocial Recovery Coaching and Core Supports such as personal care, domestic assistance, and community access. We're committed to providing exceptional care and support to our participants.

**About the Role**:
As our **NDIS Case Manager**, you'll play a vital role in ensuring the smooth and efficient operation of our services. Your responsibilities will include:

- Developing and maintaining a rapport relationship with our participants and other stakeholders.
- Developing a care plan tailored with participant's needs and conditions.
- Managing administrative tasks and documentation.
- Responding to staff and client inquiries.
- Ensuring compliance with relevant regulations and policies.
- Providing excellent customer service.

**What We're Looking For**:

- Able to communicate fluently in English and Vietnamese (desirable).
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Proficiency in Microsoft Office Suite.
- Ability to work independently and as part of a team.
- Experience in social work, welfare and community services (preferred).
- A genuine desire to support individuals with disabilities.

Pay: From $65,000.00 per year

Schedule:

- 8 hour shift
- Shift work

Supplementary Pay:

- Performance bonus
