- Provide direct care services to children, including outreach, assessment, care planning, care coordination, and crisis intervention through visits and delivery of core services based on the children's needs.
- Develop and implement care programs.
- Address the medical, social, developmental, behavioral, educational and financial needs of children to achieve optimal healthy development
- Strengthen link to providers and care settings by facilitating the arrangement of appointments, referral forms, transportations, reminders and follow-up.
- Serve children by helping to ensure that they receive screening, diagnosis and/or treatment as recommended by a health care practitioner.
- Help ensure that children are raised in a healthy, safe, and nurturing environment.
- Collaborate with all providers working with the children to ensure that their needs are being met.
- Ensures that all necessary case notes and written reports are in a timely, accurate and concise manner following established policies and procedures and time frames.
- Act as a liaison between the management and the disability support staff.
- Maintain appropriate documents, records and statistics; write reports, as needed, in an organized, timely and accurate manner.
- Ensure adhererence to policies and procedures
- work with children who need specialized care and ensure that services are coordinated to meet their assessed needs.
- Ensure thatservices are children-focused, effective ,consistent with the requirement of policy and statutory guidance
- Develop and update the disabilities related policies and procedures in accordance with relevant regulations.
- Engage in promotion of staff development and continuous learning through training, observation, coaching and reflective supervision
- Participate in continuous program planning and improvement, ensure prompt action,and work with management staff in administering action plan.
- Performs other duties as directed and necessary.
- Coordinate and schedule patient appointments, ensuring efficient use of provider time
- Collaborate with healthcare providers to ensure seamless childrens' care and follow-up

'Experience'''
- Knowledge of complex health needs in children and of the agencies and professionals engaged in assessing and providing services for children with complex health needs.
- Minimum of 2 years direct experience of working with children who have complex or additional health needs.

Desirable qualities
- Should have good interpersonal skills and have the ability to communicate verbally and in writting.
- Should have confidence in dealing with other professions and inter-agency discussions.
- commitment to team work and inter-professional collaboration.
- To work with children who need specialized care and ensure that services are coordinated to meet their assessed needs.

- Ability to proritise and manage own workload.
