Support Coordinator/ Psychosocial recovery coach required with experience. Work for a small organisation and a handful of participants to achieve NDIS goals and build capacity.
Be part of a small team and enjoy the autonomy of scheduling your own work hours, mostly from home. Some visits required every couple of months or so in Ipswich, Brisbane and South Burnett.

**Support Co-ordinator - Support Co-ordination**
As a Support Coordinator you will have responsibility for establishing and maintaining a positive collaborative relationship with each participant. You will ensure the participant is linked to the services and supports that will allow them to achieve the goals articulated in their NDIS Plan. You will be actively involved in the development, implementation, monitoring, review and reporting of participant’s services and supports alongside key stakeholders.
**Mandatory Requirements**
- Current Driver’s Licence
- Minimum Certificate IV in Disability, Mental Health or other qualification relevant to the field of human services with relevant experience
Preferred, or be willing to train
- NDIS Worker Screening Clearance
**Skills and knowledge required**
- Proven experience working with people with disability and/or mental illness.
- Demonstrated ability to network with local service providers.
- Demonstrated experience in support planning, working within a person-centred framework
- Well-developed interpersonal and communication skills, both written and verbal
- Sound organisational and administrative ability with good time-management
- Sound understanding of the NDIS, particularly Reasonable and Necessary
- Non-judgemental and ethical
- Uses initiative, is passionate and responsive
- Prioritises well and follows up tasks
- Flexible, with transferable skills
**Responsibilities**
- Identify and coordinate a range of service providers and supports to meet the identified needs of the participants accessing NDIS
- Support NDIS participants to build capacity and ensure support is focused on their goals and plans
- Actively engage in the development, monitoring, review and reporting of participant’s NDIS plans and share progress with key stakeholders
- Ensure that appropriate resources are available to achieve the agreed service delivery and related tasks
- Monitor expenditure and support participants to remain informed about their rate of expenditure and the potential effects.
- Maintain up to date, accurate and comprehensive participant records and documentation in accordance to legislative requirements.
- Liaise with and report as required to NDIA

**Psychosocial Recovery Coach - POSITION DESCRIPTION**
The role of the **Psychosocial Recovery Coach **is to assist NDIS participants to take more control of their lives and to better manage the complex challenges of day-to-day living. The Psychosocial Recovery Coach assists participants, their families, and nominees to negotiate the NDIS systems and procedures to access the best value supports to achieve their goals, become more independent and to increase social and economic participation.

**Duties and Responsibilities**
- Assisting participants to take more control of their lives and better manage complex challenges of day to day living
- Develop recovery enabling relationships
- Provide coaching to individuals to build capacity, decision making, motivation and resilience
- Working collaboratively with people, their families, carers, and other services to design, plan, implement and review a recovery plan and assist with the coordination of NDIS and other supports.
- Link participants to mainstream or community services as well as broader support systems
- Coaching the individual to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making
