The Coordinator acts as the first point of contact for individuals seeking services, guiding them through the initial stages of accessing support and fostering positive relationships between the organization and its clients or community. They are responsible for managing the initial stages of the client journey, from initial contact to assessment. The role focuses on maintaining quality standards, ensuring all documentation is current, and supporting staff through clear communication, training coordination, and oversight of rostering processes.

Supports intake processes, conducts home visits, and liaises with stakeholders to maintain high-quality, person-centred care in line with NDIS requirements and Velocity Support Services’ values.

**Duties**:

- Monitor and maintain up-to-date client records including care plans, behaviour support plans, risk assessments, and incident reports
- Facilitate onboarding of new clients, participating in intake and triage to determine service needs.
- Maintain relationships with participants, families, carers, staff, and other stakeholders, providing prompt and values-aligned customer service
- Participate in stakeholder meetings, promoting a values-aligned approach.
- Cultivate and maintain professional relationships with all staff, clients, and relevant stakeholders, ensuring interactions are respectful, constructive, and aligned with the organisation's goals.
- Complete other duties as requested and/or required by management
- Ability to cover CAS shifts if required.

**Job Type**: Part-time

Pay: $43.00 - $46.00 per hour

Expected hours: 16.5 per week

Work Location: In person
