**About the Role**

This Roaming Aboriginal Health Liaison Officer will provide patient centred care to Aboriginal clients within the tertiary and community hospital spaces across all Monash Health sites. This position will also provide support to patients within the community when discharging from Monash Health. There is a particular focus on providing cultural support and advice in clinical areas such as cardiac, renal, emergency departments and mental health. This position is responsible for a wide variety of high level functions such as cultural support and health education across key clinical and community areas. This position will also inform care coordination and case management through advocacy and negotiating for Aboriginal patients, advocating for patients. This position will work within the tertiary sector and community sector to liaise with primary care health providers to improve the Aboriginal patient journey within the service catchment area of Monash Health. The incumbent will be required to work in any clinical/community area within their approved scope of practice at any campuses of Monash Health as and when required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change. The incumbent will also be required to communicate effectively with referring and receiving health services from Victoria and across the Nation’s footprint.

**Key result areas**:

- Works as part of a team to provide health education, primary health care, individual case management care coordination and health promotion services to the Aboriginal Community
- Assesses new Aboriginal referrals, confirms contact details and liaises with other Aboriginal service providers.
- Ensures client has a GP and supports the client as required to access specialist services and medical assessment services.
- Provides health consultation and education using self-management, principles, with a community development focus.
- Provide strong leadership in areas of workload, quality improvement and professional development
- Enhances access to health services for community members when required.
- Actively encourages and supports clients/families/communities to engage in activities which maximise health gain.
- Uses appropriate clinical skills for detection of health issues, monitoring those with diagnosed health problems and supporting their access to health services.
- Participates in a multidisciplinary health service to ensure patient-centred care.
- Can work across any of Monash Health hospital sites to meet patient demands.
- Able to work a seven day a week roster.
- Work with indirect or remote supervision to implement related activities
- Maintains appropriate, legible records and ensure security and confidentiality of client information.
- Accurately records data electronically, collates and reports workload statistics.
