**About Northern Beaches Hospital**
Operated by Healthscope, Northern Beaches Hospital (NBH) is located in Frenchs Forest, only 13kms north of Sydney CBD. The hospital is a contemporary 488 bed facility comprising of 60% public and 40% private beds. Providing a wide range of health services to the local community, including Surgery, a 50 bed Emergency Department, Intensive Care, Birthing, Paediatrics, Cardiac, Mental Health, Dialysis, Cancer Centre and Outpatients.
NBH has 13 operating theatres, one hybrid/cardiac catheter theatre, two Catheter Laboratories and four endoscopy procedure rooms, all supported by a critical care unit which includes two intensive care wards.

**About the role**
Northern Beaches Hospital is seeking a motivated and passionate nurse to work within our **patient flow team** as a **clinical nurse specialist** focused on discharge/transfer of care.
You will be patient-focused and able to work across our hospital to ensure that patients have a clear discharge or transfer of care plan. This will include complex care planning, liaising with the multidisciplinary team and ensuring that referrals are made to other facilities in a timely manner.
You will work well as a team member, whilst also being a self-starter, and able to manage multiple priorities.

In this role, you will have:

- Registration as a Nurse (level 1) with the Australian Health Practitioner Regulation Agency (AHPRA)
- At least 3-4 years of relevant clinical experience

Desirable Qualifications:

- Post Graduate qualification in a relevant specialty or an equivalent level of experience
- Experience working in the private health system
